The CMS Form Tab includes information for several boxes on the CMS-1500 form. The data entered on this window will appear on the CMS-1500 in boxes 9, 13, 19, 24e, 24i, 24j, 25, 32b, and 33b. These entries may vary by insurance carrier, so be sure that the data is entered correctly for this carrier.
